OVERVIEW

As an integral member of an inter-professional team, the Social Worker (SW) for the Mental Health Services cares for individuals with mental illness. The Social Worker will have a split role overseeing central intake and the urgent care clinic in the Adult mental health outpatient program and Emergency Department (ED).

In this role the SW conducts brief intake assessments on clients referred to the Outpatient mental health program to determine appropriateness of service for client. The Social Worker will also work closely with the Urgent Care Clinic Psychiatrists to conduct assessment, stabilization and treatment/ follow up for individuals requiring urgent follow up. The Social Worker will work with other clinicians in the Mental Health Services, East Toronto Health Partners (ETHP), Primary care providers and the community to connect client to services and safely discharge from the Urgent care clinic.

 

EDUCATION

-  Master of Social Work from an accredited school of social work, or equivalent degree, as recognized by the Ontario College of Registered Social Workers and Social Service Workers (OCSWSSW).
- Must be a member of the OCSWSSW in good standing.

 

QUALIFICATIONS

- A minimum of 5 years experience in a Mental Health/ Psychiatric services environment; Knowledge and experience in mental health assessments and intervention required.
- Demonstrated experience in intervention with high-risk populations.
- Demonstrated excellence in crisis intervention clinical skills including mental health assessments, risk assessments, and safety plans required.
- Demonstrated knowledge of the Mental Health Act, Mental Health Reform, Privacy and Consent to Treat Act and issues related to same required.
- Demonstrated experience in applying the principles of crisis counselling in clinical practice required.
- Demonstrated experience in developing community linkages and liaisons.
- Demonstrated excellence in problem solving techniques, conflict resolution, and a willingness to contribution to high performing teams.
