OVERVIEW

Reporting to the Manager, Medical and Surgical Clinics, the Clinical Specialist (CS) - Diabetes will provide clinical leadership, conduct and communicate research, provide expert consultation and education for Nursing, Dietitians and other members of the care team for a specialized client population.
    
The CS has a passion for diabetes care and will play a lead role in the implementation of best practices and clinical guidelines, promote the use of evidence-based interventions and facilitate system change in relation to the complex care issues of the patient population.  
    
This clinician leader will have demonstrated knowledge, skill and ability relevant to addressing complexities of diabetes care in the acute care setting through collaborative team consultation and education, ensuring proactive inpatient care, outpatient and community-follow-up, with the goal of preventing complications and re-admissions to hospital.  
    
With in-depth knowledge and skills, advanced judgment and clinical experience the CS - Diabetes assists in providing solutions for complex health system issues at all levels with patients, families, other disciplines, administrators and policy-makers.  
    
The role promotes excellence in clinical diabetes practice and advocates for excellence in both practice and research.  The CS - Diabetes improves access to effective, integrated, interdisciplinary and coordinated care at Michael Garron Hospital, and works with system-wide networks to benefit the health of our wider community.

 

EDUCATION

-  Specialty Certification in Diabetes (CDE), > 5 years, demonstrates current Certification, required.
-  Completion of a professional degree in Nursing, Dietetics or other regulated health care profession.
-  Registration with the applicable professional regulatory body (e.g., College of Nurses of Ontario, College of Dietitians of Ontario, etc.) 
-  Member of professional organization(s) in relevant profession such as RNAO.
-  Current BLS certificate required.


QUALIFICATIONS

-  Minimum 5 years relevant clinical specialty-related experience (diabetes education) in an acute care hospital setting.
-  Experience working in an advanced clinical practice role or leadership role required.
-  Demonstrated in-depth knowledge of diabetes and patient self-management support. 
-  Demonstrated proficiency with insulin pump teaching and pump management. 
-  Knowledge and skill in regard to patient safety, change leadership, project management and evaluation an asset.
-  Demonstrated ability to work in a self-directed manner using critical thinking and problem-solving skills.
-  Demonstrated sound interpersonal, team development and conflict management skills. 
-  Demonstrated knowledge in research methods, and experience originating and conducting research.
-  Evidence of sound knowledge and skill in adult education theory and techniques.
-  Knowledge of models of care regarding interprofessional practice and education. 
-  Demonstrated flexibility to work occasional shifts and weekends to support operational needs.
-  Excellent work and attendance record. 
-  Proficient with Microsoft office and other data processing software.
